The bills/vouchers/reports should contain your name as the one who underwent the treatment, your illness type, details of individual items of medical treatment provided and treatment dates.
The prescriptions should clearly display the prescribed medicines.
The pharmacy bill should clearly display the price and should bear the pharmacy stamp.
For dental treatment, the bills/vouchers/reports should provide the details of the treatment performed and the tooth treated.
For Repatriation of Mortal Remains: In case of burial abroad or repatriation of mortal remains, documents to be submitted include the official death certificate, physician’s statement providing the cause of death and original receipts/bills of expenses incurred.
For Medical Evacuation: To be transported to India on medical grounds, a medical statement from a registered physician stating the cause of the ailment and the need for transportation, as well as original receipts/bills of expenses incurred, is required.
For Passport Loss: A Police Report that is obtained within 24 hours of you being aware of the theft and original receipts/bills for expenses incurred for a fresh/new passport.
For Total Baggage Loss: Documents to be submitted along with your completed claim form include a Property Irregularity Report or other report that is generally issued by common carriers in the event of checked-in baggage loss and a letter from the airlines mentioning compensation for the lost baggage.
For Personal Accident: Original bills/vouchers/reports/discharge summary of you being treated, the cause of the accident, details of individual items of medical treatment provided, along with dates of treatment. A Post Mortem Report must be provided in case of death.
For Personal Liability: Proof of A Court’s judicial decision must be provided.
For Financial Emergency Assistance: A Police Report filed within 24 hours of you being aware of the robbery.
For Trip Delay: Doctor’s statement and medical report or Police Report confirming the trip delay. If the delay is caused by illness/death of the travelling companion,
Original tickets of the insured and travelling companion indicating travel on the same date to the same destination. It should contain your passport number.
Original bills/receipts of extra expenses and proof of charges for cancellation that the carrier levied.
For Trip Interruption and Cancellation:
For Medical Reasons – A doctor's statement and medical reports must be submitted.
