  • Name of the policyholder or the insured for whom the claim is being filed. 
  • Type of disease.
  • Name and address of attending physician and hospital. 
  • Date of medical procedures. 
  • Any other information requested by our claims settlement team.

Note that it is important to intimate your critical illness claim within 7 days of receiving your initial diagnosis results. Any type of delay in claim intimation can lead to denial.

Step 2: Get Your Treatment Done

Once your claim request is recorded, you can get your critical illness treatment done on the desired dates. Make sure to save all the medical invoices, receipts and bills, as they will be used for the claim process.

Step 3: Claim Procedure

Fill out the critical illness insurance claim form and submit it along with the original documents relating to your hospitalisation or treatment to RCARE Health.

Make sure to submit these documents immediately but not more than 15 days after your discharge.
