Checklist of mandatory documents required while filing claim request for Hospitalisation, Domiciliary Hospitalisation & Critical Illness:
Hospitalisation / Day-Care Treatment
- Completely filled and duly signed Claim Form
- Doctor’s first prescription (with commencement date of the symptom of disease)
- Treatment papers (along with doctors prescriptions)
- Investigation reports (X-ray/Scan/ECG, Laboratory etc)
- Original medical bills and receipts(from hospital, doctors, medical shops or diagnostic centre, all supported by doctor's advice)
- Original hospital discharge card
- Copy of FIR (in case of accident)
- Cancelled Cheque – CTS 2010 format (Printed account number, IFSC code and Printed name of insured). In case, insured’s name is not printed on the canceled cheque, provide scanned copy of 1st page of passbook or bank statement.
- Photocopy of identification card of patient & PAN card of insured
- Copy of Health card
Please note all documents should be originalCritical Illness Claims
- Completely filled and duly signed Claim Form
- Consultant’s certificate with diagnosis (including date when symptoms first occurred)
- Relevant Investigation reports (Radiology, Pathology etc) confirming the diagnosis
- Original medical bills and receipts (from hospital, doctors, medical shops or diagnostic centre, all supported by doctor's advice)
- Hospital admission & discharge card/ certificate
- Cancelled Cheque – CTS 2010 format (Printed account number, IFSC code and Printed name of insured). In case, insured’s name is not printed on the canceled cheque, provide scanned copy of 1st page of passbook or bank statement.
- Photocopy of identification card of patient & PAN card of insured

- Certificate from attending doctor/physician stating the condition of the patient does not allow him/her to be removed to hospital/nursing home (if this is the case)
- Documentary proof showing lack of accommodation in hospital/nursing home (if this is the case)
- Cancelled Cheque – CTS 2010 format (Printed account number, IFSC code and Printed name of insured). In case, insured’s name is not printed on the canceled cheque, provide scanned copy of 1st page of passbook or bank statement
- Photocopy of identification card of patient & PAN card of insured

- All documents should be original- The above list is only indicative / claimant may have to submit the scan copies of original documents additional / information if required
- Reliance General Insurance company limited will reserve the rights to seek all the original documents in physical form for a period of one year from the date of claim submission.
